Youth Services Admissions Packet
To make a referral for our therapeutic group home or day treatment program, a clinical mental health assessment that has occurred within the past year may be faxed to admissions. Admissions will be able to assist in obtaining clinical information if not readily available and can assist with scheduling mental health assessment if needed. To obtain additional information about our programs, services, and fees please do not hesitate to contact admissions.Â
